program summary:Life skills education and case management program for aging-out youth who are active with DCP&P or who have been open and in out-of-home placement. Program offers independent living skills training youth living in Warren, Hunterdon or Somerset Counties. There are no fees for this service.
Eligibility Criteria:
Must be a resident of Warren, Hunterdon or Somerset County; be between 16-21; be in or have had an out-of home placement; and be ready for transition to the community or aging out of the DCP&P system.
